What causes arcing inside a microwave, from harmless to serious

A burnt waveguide cover. The most common genuine fault. Grease and food splatter soak into it over time, then carbonise, and carbon conducts. Once it starts arcing it burns through progressively. It is an inexpensive part and easy to replace, but ignoring it lets the arcing reach the waveguide and the magnetron antenna behind, which is a much bigger bill.
Damaged cavity paint. The interior coating is not cosmetic; it insulates the steel underneath. Where it has chipped or worn through -- often at the bottom or around the door frame -- bare metal is exposed and can arc, and it then rusts. Small areas can be repaired with proper microwave-safe cavity paint. Extensive rust-through means the appliance is finished.
Rack supports and accessories. Metal racks supplied with convection models are fine in convection mode but can arc in microwave mode if positioned wrongly or if their protective supports are missing.
A failed magnetron antenna cap. Less common, and more serious. Arcing at the waveguide entrance can be the antenna cap itself having burnt.
